    Summary: We found fault with the Council delaying for two and a half months outside the statutory timescales in producing an amended final Education, Health and Care Plan for Ms X’s child. We also found fault with the Council failing to consider the suitability of Ms X’s child’s access to education from February 2025 to May 2025. The Council’s actions caused Ms X frustration, uncertainty and delayed her appeal rights. The Council agreed to apologise to Ms X and provide her with a symbolic payment for the injustice caused by its fault.

It properly considered the alternative educational provision suitable for Ms X’s son when he had no school place, but it should have started organising this sooner. The Council did not fully consider whether it could implement his Education Health and Care Plan while he was not at school and it did not always communicate with Ms X sufficiently. The Council tried to find Ms X’s son a school place, but when it could not, this was service failure. The Council’s shortcomings caused Ms X distress, uncertainty and frustration. The Council has made some improvements to its service. It has agreed to also apologise to Ms X and make a symbolic payment to her in recognition of the impact on her.

    Summary: Ms X complained about the Council’s handling of long-standing anti-social behaviour concerns and about the Council’s handling of her housing application. Ms X also complained the Council did not have regard for her vulnerabilities. We have found the Council at fault for failing to consider acting earlier than it did at key points, and for some delays in completing reviews. We have also found the Council at fault for some of its communication. These faults caused avoidable uncertainty and distress for Ms X. We have not found fault with the conduct of the ASB case review in 2025. We cannot therefore question the outcome. The Council has agreed to apologise to Ms X, correct inaccurate information provided to other agencies, and pay Ms X a financial remedy to recognise the uncertainty caused.

    Summary: Mr X complained the Council did not tell the family in advance that Mrs Y’s care and support would be chargeable from day one after she was discharged from hospital. This affected Mrs Y financially and caused distress. The Council was at fault for failing to follow its procedures on reablement support, and failing to provide proper advice to Mrs Y and her family. The Council agreed to reimburse Mrs Y’s care fees and provide a symbolic financial remedy for Mr X’s time and trouble.
