Summary: Miss D complains the Council placed her in unsuitable homelessness accommodation. I have found fault by the Council. Miss D remained in bed and breakfast style accommodation for 24 weeks over the allowed time limit and was subsequently in unsuitable temporary accommodation for seven months. The Council has agreed to pay financial redress for the time Miss D was deprived of suitable accommodation.

  • Milton Keynes Council (25 006 747)

Statement Upheld Special educational needs 23-Feb-2026

Summary: Mr X complained about the Council’s handling of his child, Y’s education. The Council was at fault for failing to maintain oversight and secure therapy provision as set out in Y’s final Education, Health and Care (EHC) Plan between April and September 2025 after a SEND tribunal order. The Council agreed to apologise and make a payment to acknowledge Y’s loss of provision and for the injustice caused to both Mr X and Y.

Statement Closed after initial enquiries Special educational needs 23-Feb-2026

Summary: We will not investigate this complaint about delays in completing an Education Health and Care Plan annual review. This is because the complaint is late, and there are no good reasons to exercise discretion to investigate now.

  • Surecare (Croydon and Sutton) (25 012 250)

Statement Closed after initial enquiries Domiciliary care 23-Feb-2026

Summary: We will not investigate this complaint about privately arranged adult social care at home. This is because the injustice, frustration at not being listened to, is not enough to justify our involvement. It is also unlikely an investigation would reach a different outcome. The complainant wanted the Care Provider to withdraw its service, and that has since happened.

  • Walsall Metropolitan Borough Council (25 012 481)

Statement Closed after initial enquiries Planning advice 23-Feb-2026

Summary: We will not investigate this complaint that the Council advised Mrs X to apply for planning permission for change of use. The complaint is late and there is no reason why Mrs X could not have contacted us much sooner. Also it is reasonable to expect her to complain to the Information Commissioner it she believes the Council is withholding information.

  • London Borough of Islington (24 007 618)

Summary: Mr B complained that the London Borough of Islington, North Central London ICB and North London NHS Foundation Trust did not assess or meet Mrs X’s aftercare needs after she was detained under the Mental Health Act 1983. We find fault with the Council, the ICB and the Trust. They should have completed a comprehensive assessment of Mrs X’s needs when her detention ended, and should have produced a clear plan of how all her needs would be met. The failure to do so has caused confusion, stress and frustration. The organisations have agreed to take action to address the injustice.
