Summary: Mrs X complained the Council failed to provide her child, Y, with all the provision set out in their Education Health and Care Plan. The Council was at fault as it failed to provide Y with the play therapy, speech and language therapy and teaching assistant support set out in their Plan between September and November 2023. The Council has agreed to apologise and pay Mrs X £600 to acknowledge the missed provision and frustration this caused.

Summary: Mr X complained about the way Transport for London (TfL) dealt with his application for a grant under its car scrappage scheme. We have not found fault by TfL in the way it decided to reject Mr X’s application. But we have found fault by TfL in failing to communicate properly with Mr X about his application, and failing to publish changes to its scheme in a timely fashion, causing injustice. We have recommended that TfL apologises to Mr X, and makes him a symbolic payment to reflect his time, trouble, and the loss of opportunity to sell his car sooner.

Summary: The Council failed to act promptly to provide Ms X with temporary accommodation several times and when it did it, the accommodation it provided was often unsuitable. The Council failed to support Ms X properly while she and her children were being evicted and again when they needed to flee domestic abuse. Its communication with Ms X was poor and it significantly delayed carrying out reviews into its homelessness decisions. To recognise the injustice caused, including the period Ms X and her children spent in unsuitable accommodation, the Council has agreed to apologise, pay Ms X £3,375 and carry out service improvements.

It placed the family in bed and breakfast accommodation for over 41 weeks, 35 weeks over the maximum time such accommodation can be used for homeless applicants with dependent children. The Council has agreed to make a payment to Mr B to remedy his family’s injustice. It has also agreed to make service improvements.
