- The Council was at fault in the way it dealt with assessing a child for an Education, Health and Care Plan. The Council has agreed to issue guidance to staff telling them that children not previously known to the Council's children's social care service should be assessed by children's social care when assessing their educational, health and care needs.

- The Council agreed to provide the Ombudsman with evidence of how it carried out the actions it identified at the conclusion of its complaint investigation. This included a review of the appropriateness of referrals to alternative provision placements and how it ensures young people receive their entitlement to full time education. It also included ensuring the Council's SEN panel properly considers all relevant information and communicates with parents during the process.
- The Council agreed to carry out an audit of 10% of all children with EHC Plans ensuring there has been an up to date review or whether there are any delays in issuing a final amended EHC Plan. The sample must include a number of children receiving alternative provision because of exclusion, illness or otherwise.
