London Borough of Brent (24 021 318)
Statement Upheld Special educational needs 28-Oct-2025
Summary: The Council has accepted that it failed to provide the special educational provision for the complainant’s brother. We agree with the Council’s findings, but we considered the Council had not fully recognised the injustice caused by the loss of education for the brother and avoidable distress to the family. The Council has now agreed to make a symbolic payment for this injustice. We are therefore closing the complaint.

Coventry City Council (24 023 422)
Statement Closed after initial enquiries Other 28-Oct-2025
Summary: We will not investigate Mrs M’s complaint about a social care assessment for her daughter, G, because we could not add to the Council’s response, and further investigation would not lead to a different outcome. There is insufficient outstanding injustice to justify our involvement.

Birmingham City Council (25 000 338)
Statement Upheld Allocations 28-Oct-2025
Summary: Mrs X complained about the Council closing her housing application. We find that the Council failed to provide clear information about the documents she needed to submit to support her application, or the evidence it would accept as proof of her address. It then closed Mrs X’s application as incomplete without specifying what information she had not provided. This caused Mrs X avoidable frustration and inconvenience and delayed her ability to join the housing register. The Council has agreed to assess Mrs X’s application and, if she qualifies to join the register, it will backdate her award and registration date. The Council has also agreed to apologise and make a symbolic payment to Mrs X.

London Borough of Newham (25 001 044)
Statement Upheld Parking and other penalties 28-Oct-2025
Summary: Mr X complains the Council did not properly deal with a Penalty Charge Notice (PCN) causing financial loss. The Council gave Mr X contradictory information and passed Mr X’s case to enforcement agents before a deadline to pay had expired. Mr X suffered financial loss. The Council will apologise, allow Mr X to pay the PCN at the original reduced rate, pay Mr X £200 for time and trouble, ensure he is not liable for any enforcement charges in relation to this PCN and train relevant staff.

London Borough of Haringey (25 002 280)
Statement Closed after initial enquiries Child protection 28-Oct-2025
Summary: We will not investigate Ms X’s complaint about children services’ actions. We are unlikely to find fault in the Council’s decision not to accommodate Y. The Information Commissioner’s Office is better placed to consider data protection complaints, and we are unlikely to achieve more than the Council ending child protection plans.

Essex County Council (24 004 328)
Statement Upheld Alternative provision 28-Oct-2025
Summary: Miss X complained about the Council’s failure to ensure her son (Y) received education and support included in his Education Health and Care Plan. We found fault with the Council in the way it carried out its alternative provision duty and reviewed Y’s Educational Health and Care Plan. The Council’s fault meant Y missed education and support he needed. It also caused significant distress to Miss X. The Council has agreed to apologise, arrange a meeting to check Y’s engagement with his post-16 education and make symbolic payments. The Council has previously agreed to improve its services.

Summary: Mr X complained the Council failed to properly allocate address numbering to a development and about how it dealt with his complaint. Leeds City Council (24 017 661)
Statement Upheld Other 28-Oct-2025
Summary: Miss D complained on behalf of her sister who lives in supported living accommodation arranged by the Council. We upheld her complaint, finding the Council failed to take sufficient action to safeguard Miss D’s sister after she experienced aggressive behaviour from another resident. We considered its inattention caused unnecessary distress to both Miss D and her sister. The Council has accepted these findings and at the end of this statement, we set out action it has agreed to take to remedy their injustice and try to prevent a repeat.
