    Summary: We investigated a complaint about arranging care for Mr Y after he left hospital. We found fault with the Council; there were unexplained delays when it was searching for a nursing home placement for Mr Y in November 2021. The Council also did not investigate Dr X’s complaint in line with guidance and provided responses which were unclear, contradictory and did not take account of all the facts. Dr X was caused unnecessary frustration when dealing with the Council. We recommended an apology, service improvements and a financial remedy to address this injustice. We found no fault with the actions of the Trust or the ICB.

    Summary: Miss A complained her assigned care coordinator at the Trust failed to arrange changes to her flat and failed to arrange suitable treatment for her. We found no fault with the Trust’s actions. Arranging repairs to Miss A’s property was not the Trust’s responsibility and the treatment unit was closed due to the COVID-19 pandemic.

    Summary: The Ombudsmen will not investigate a complaint about social care provided under section 117 of the Mental Health Act. This is because the information we have seen suggests the assessment the Council completed was in line with expected standards and we are unlikely to find fault.

    Summary: Mr T complained about the decision to terminate his daughter’s, Miss C’s, placement in specialist residential accommodation commissioned by the Council and the Integrated Care Board (ICB) as part of her aftercare services. He also complained about the lack of mental health support for Miss C while in the placement. We did not find fault by the Council and the ICB for the mental health support offered or the decision to terminate the placement. However, the authorities failed to have a contingency plan in place and failed to provide move-on accommodation as aftercare. The faults caused avoidable distress to Miss C and her parents. They have also been left with quantifiable financial losses because of the faults. We made recommendations to remedy the injustice caused and improve services.

    Summary: We found fault by a Trust regarding the discharge planning for Mr F, a young man who had been detained under the Mental Health Act 1983. We also found fault by a Council and Integrated Care Board as there was avoidable confusion about which agencies were responsible for the aftercare to which Mr F was entitled. This fault delayed Mr F’s discharge and caused him distress. These organisations will provide Mr F with a financial remedy and act to prevent similar problems occurring in future.
