Statement Upheld Hospital acute services 25-Jan-2021
Summary: We have partially upheld Mr S's complaint about his father's discharge from hospital to a residential care home. The Council and Trust have already apologised to Mr S and we consider this a suitable remedy for distress caused to Mr S. The Council says it has already improved its way of working following Mr S's complaint. We have asked for evidence of these improvements and recommended further service improvements. The Council and Trust accept our recommendations, so we have completed our investigation.

Summary: We found fault by the Council with regards to the care and support it provided to Miss X, a woman with complex needs, in the community. We also found fault by the Council and Trust concerning the handling of Miss X's discharge from hospital. This fault caused Miss X significant distress and frustration. The Council and Trust have agreed to apologise to Miss X and the Council will make a payment in recognition of the distress she suffered.

Summary: We will not investigate Mr L's complaint about arrangements for his mother's discharge from hospital in November 2018. The complaint is late and we have not seen sufficient reasons to investigate it now.
Statement Not upheld Hospital acute services 20-Oct-2020
Summary: Ms A complains that her mother, Mrs B, was unsafely discharged from hospital before she was ready, and without the right care in place at home. The Ombudsmen find no fault in the decision that Mrs B was ready for discharge from hospital. However, the Council has accepted there was fault in its arrangements for Mrs B's care once she returned home and it has provided an appropriate remedy.

Summary: The Ombudsman will not investigate Ms X's complaint. This is because the Parliamentary and Health Service Ombudsman is better placed to consider her complaint.
Statement Not upheld Hospital acute services 02-Oct-2020
Summary: Ms D complained about information she received about her mother's discharge from hospital to residential care and associated charges. The Ombudsmen have upheld Ms D's complaints about Manchester City Council. We have not upheld her complaints about Pennine Acute Hospitals NHS Trust and Manchester University NHS Foundation Trust. The Council has accepted our recommendations, so we have completed our investigation.

Summary: Mr E complains about the actions of a Council and a Trust in relation to child protection concerns and a child protection examination carried out on his son. While there were faults, I do not consider these led to the injustice that Mr E has claimed. In addition, both organisations carried out appropriate work to try and remedy the situation and prevent similar occurrences.

Summary: We find the Trust (NLaG) delayed in processing a request for an incontinence assessment but we do not find evidence of injustice. The care home's records show adequate continence care provision and we note the Trust's general service improvement in shifting assessment services to its district nurses.

Summary: Ms Q complains about the care her father received. The Ombudsmen have found no fault by the Council. While there was fault by the Trust and the Care Home, those organisations have taken appropriate steps to remedy the injustice Ms Q and Mr R suffered.

Summary: Mrs Z complains about her late father's care and assessments in hospital, discharge from hospital and communication by the organisations involved. We have found fault relating to documentation, communication and complaint handling. We have not upheld the rest of the complaint. The Council has offered a financial remedy for Mrs Z which we consider is fair. We have recommended apologies for Mrs Z and service improvements. The organisations concerned accept our recommendations. We have therefore completed our investigation.
