    Summary: Mr X complained about the actions of the Council when the care home his mother lived in raised a safeguarding alert. Mr X said this caused him distress and his mother’s health to deteriorate, eventually leading to her death. The Council carried out a satisfactory investigation which identified fault in its own actions and those of the care home. It made recommendations to prevent a recurrence of the issues complained about.

    Summary: Mr X complained for his father, Mr Y, about the standard of care at the Grange Nursing Home (the Grange) and the Council’s decision to end one to one support. The Ombudsman did not find fault in the Council’s decision to stop one to one support or in the care provided by the Grange. There was some fault in the Council’s record keeping.

    Summary: We have upheld some of Ms X’s complaints about a council reducing her brother’s care. We found the council took too long to complete an assessment and was not open with the family when safeguarding issues were raised. This was fault which caused avoidable distress. To remedy the injustice, the council will apologise and make a symbolic payment as described in this statement.

    Summary: We will not investigate Ms X’s complaint the Council included incorrect information in her son’s Deprivation of Liberty Safeguards authorisation. This is because there is insufficient evidence of a significant personal injustice to Mr Y, and the Information Commissioner’s Office is best placed to consider complaints about data rectification.

    Summary: We will not investigate Ms X’s complaints the Council wilfully disregarded legal advice, shared the wrong version of our previous decision, made defamatory comments about her and failed to follow a court order. Part of the complaint is about matters we already considered in 2017 and, despite Ms X since finding out new information, reinvestigation is not warranted. Other parts of the complaint are about information shared in court, which we cannot consider, and issues that would best be dealt with in court as part of the ongoing proceedings.
