    Summary: Mr X complained about the Council’s failure to notify him of a change in benefit entitlement when acting as his wife’s appointee and related matters. This led to an overpayment of other benefits. During the course of the Ombudsman’s investigation, the Council accepted it was at fault and offered to make a payment to Mr X and correct any financial injustice caused. The Council has also agreed to apologise to Mr and Mrs X.

    Summary: The Ombudsman will not investigate Ms B’s complaint about the Council’s adult social care service. This is because the Council has recognised some service failings and apologised. Given the lapse of time, we cannot safely say the level of loss, harm and distress Ms B has suffered from the unresolved issues is significant enough to justify our involvement.

    Summary: The Ombudsmen found no fault by the Council, Trust or CCG with regards to the care and support they provided to a woman with mental health problems. The Ombudsmen did find fault with a risk assessment the Trust completed. However, we are satisfied this did not have a significant impact on the care the Trust provided.

    Summary: Mrs B complains about the amount of money the Council asks her husband to pay towards his care. We uphold the complaint, finding the Council has not properly assessed disability related expenses that Mr B might have. This causes uncertainty for Mr and Mrs B. We recommend the Council apologise, write off some charges incurred by Mr B and reassess his financial contribution following advice set out in this statement.

    Summary: Mr Y complains the Council failed to provide him with information about funding for his father’s (Mr X) residential care. This is because there is no evidence of fault causing an injustice to either Ms A or Mr B warranting an Ombudsman investigation and any further investigation by the Ombudsman could not add to the Council’s responses or make a different finding of the kind Ms A wants.

    Summary: Mr B complains the Council has not properly reviewed and increased his care package. Mr B says the Council has failed to appoint an advocate, comply with agreed methods of contact and make interim payments pending a re-assessment. Mr B says this has caused him significant distress and puts his health at risk. The Ombudsman does not find fault in how the Council completed Mr B’s re-assessment.

    Summary: The Ombudsmen do not agree Haringey Clinical Commissioning Group should have commissioned a day care centre to meet Q’s needs during the holiday period. The Ombudsmen do not consider there was fault in the way London Borough of Haringey carried out Mr and Mrs P’s carers assessments. However, it should have completed them when it was aware Q’s care package would significantly change.
