    Summary: Mrs Y complains about the Council's lack of transition planning for her son, X, who has recently turned 18. The Ombudsman finds the Council failed to appropriately forward plan for X's transition into adult services. This is not in accordance with the requirements of the law and guidance. The Council will remedy the injustice this caused with the actions listed at the end of this statement.

The Ombudsman finds there was fault in how the Council handled Mr X's transition. This caused Mr X and those caring for him understandable uncertainty. The Council has accepted our recommendation to apologise, pay a financial remedy to Mr X and take measures to prevent it happening again.

    Summary: Ms T complains about the way the Council handled her son, C's, transition from children to adult services. The Council was at fault because it delayed in providing C's support plan to the care provider but this did not cause him a significant injustice. There was also fault in that C's support worker failed to tell him about an appointment in respect of his claim for personal independence payment which caused him distress and inconvenience because he did not receive payments for several weeks. The remainder of Ms T's complaints are not upheld.
