    Summary: The Ombudsmen find fault with the way the Council conducted a child safeguarding investigation and with its failure to share information in a timely manner. The Ombudsmen also find fault with the documentation the Trust provided to inform the safeguarding process.

    Summary: The Ombudsmen found fault by the Council and a Trust in the way they dealt with the complainant's late mother's discharge from hospital and then provided care and support to her at home. The Council and the Trust also failed to provide suitable care and support to prevent the complainant's mother developing pressure ulcers and this contributed to a decline in her wellbeing. The Council and the Trust have agreed to the Ombudsmen's recommendations that they apologise to the complainant and pay £5,000 to her mother's estate. The Council has also agreed to pay the complainant £250 for her time and trouble in dealing with the complaint.

    Summary: The Ombudsmen found no fault with the way in which the Trust and Council supported a young man with Asperger's Syndrome and associated mental health problems.

    Summary: There is no evidence to show a Medical Centre falsified records to cover up allegations of poor care in a care home. There is no fault in the way the Council investigated a safeguarding alert and it found fault by the care home. However the Council did not remedy the injustice caused to the complainant. The Ombudsmen recommend a written apology and a financial remedy for the complainant.

    Summary: The Ombudsmen find no fault in the Trust's decision not to respond to an email in September 2013. Further, the Ombudsmen found no fault in the decision to make a safeguarding alert in December 2013, or in the Trust's decision not to arrange a meeting after April 2014.

    Summary: Mr J complained about the Council's failure to support him following his discharge from hospital. There is no evidence of fault as the Council put services in place to meet Mr J's needs. Mr J also complained he was discharged from hospital too soon. However, he was reviewed regularly and appropriate discharge arrangements made. There is no evidence of fault in the CCG's decision to decline funding for a specific service Mr J requested.
