    Summary: The Council is at fault as it failed to properly consider whether Miss X had fluctuating needs when carrying out care needs assessments so wrongly recommended her personal budget be reduced. The Council is also at fault as it cannot demonstrate it considered all Miss X’s claimed disability related expenditure. The faults by the Council caused significant distress and uncertainty to Miss X and put her to avoidable time and trouble which the Council has agreed to remedy.

    Summary: Mrs X complained about the Council’s management of her daughter’s (Miss D’s) support. There is fault in way the Council handled D’s financial support. This caused distress for Miss D and Mrs X. The Council has agreed to remedy this injustice.

    Statement Upheld Safeguarding 04-Sep-2019

    Summary: The Ombudsmen found faults in safeguarding Mrs D’s mother Mrs F, assessing Mrs F and her husband, and in responding to Mrs D’s complaint. The Council and Trust have already accepted there were faults and apologised. This is an appropriate way to address the injustice to Mrs D’s family. The Council and Trust have also improved services since Mrs D’s complaint. Some of their process and information remain flawed, so we have recommended service improvements. The Council and Trust accept our recommendations, so the Ombudsmen have completed their investigation.

    Summary: The Council and Trust have already acknowledged they failed to assess Mr F as a carer and failed to provide him with support. The Council has accepted our recommendation to provide an apology. The Trust has accepted our recommendation to issue staff guidance. The Ombudsmen have therefore completed their investigation.

    Statement Upheld Safeguarding 31-Oct-2019

    Summary: When the Council was notified about concerns for Mr B’s welfare, it failed to properly consider whether to carry out an assessment of Mr B’s care and support needs and did not always properly consider whether to make safeguarding enquiries. The Council has agreed to apologise and make a payment to Mr B, and to take action to prevent similar failings in future.

    Statement Upheld School transport 11-Feb-2020

    Summary: although the Council acknowledged it was at fault in April 2019 for failing to provide school transport for Mrs M’s daughter, G, and promised to take action, the Council did not put matters right. Mrs M has suffered further injustice. I have recommended a remedy.

    Summary: Ms X complains about the Council’s involvement in her family over child protection matters. The Ombudsman finds fault with how the Council communicated and provided support to Ms X. We have recommended the Council makes financial payment to Ms X.
