    Summary: Mrs M complained on behalf of herself and her adult daughter, Miss D. She complained about faults by City of Bradford Metropolitan District Council and NHS West Yorkshire Integrated Care Board leading to Miss D being unable to go to college and missing out on special educational provision. She also complained about poor communication by the organisations. We have upheld the complaint and found the faults caused significant injustice to Miss D and Mrs M. This includes Miss D missing all her education and essential special educational needs support for about 12 months. Mrs M could not take her usual breaks from her caring role, leaving her exhausted. The organisations’ faults also caused Mrs M avoidable distress and time and trouble. The organisations accepted our recommendations, so we have completed our investigation.

Ms M complained the organisations failed to ensure her child, S, received therapy provision set out in his Education, Health and Care (EHC) Plan. She also complained about the Council’s and Trust’s complaint handling. We have upheld parts of Ms M’s complaint relating to therapy provision and complaint handling. The organisations have accepted our recommendations. We have now completed our investigation.

    Summary: Mr X complained about Derbyshire County Council and NHS Derby and Derbyshire Integrated Care Board. We will not consider his complaint about the delay issuing his son’s Education, Health and Care Plan. That complaint is late. Also, we cannot consider Mr X’s complaint about not securing health provision to enable Y to attend school. That is bound up with matters which Mr X appealed to the First-tier Tribunal (Special Educational Needs and Disability), which places the complaint out of jurisdiction.

    Summary: Ms X complains about delays by the Council and the Trust in completing an autism assessment for her child and for not fast‑tracking their assessment. We found fault with the initial referral, but the Trust has already remedied this. We also found fault with the triage panel meeting that led to some delay. We did not find fault by the Council and the Trust with how they considered Ms X’s fast‑track request. However, fault with a panel meeting led to some uncertainty about whether this could have been considered sooner. We have recommended the Trust and the Council take actions to address the faults and apologise for the distress caused to Ms X.
