We find the Council was at fault as it delayed communicating with the school to resolve the issue with the assisted listening device. It also did not adhere to statutory timescales when dealing with Mr F’s daughter’s annual review and it was at fault for its overall communication with Mr F. The Council has agreed to our recommendations to address the injustice caused by fault.
