We have found no evidence of fault in the way the Council assessed and supported Mr Y’s care and support needs and communicated with Ms X. We found fault by the Council as it delayed carrying out a review of Mr Y’s care and support plan. But this did not cause a significant injustice to Mr Y who was continuing to receive care and support from the Council. So, we have completed our investigation.
