We found the Council and its appeal panel acted without fault.

Investigation into this complaint has been discontinued.

We have discontinued our investigation as it would not achieve any useful outcome for Mrs X and her daughter.

The Council will apologise, make a payment to recognise the impact of the missed provision and take action to ensure the identified fault does not occur again in future.
