    Summary: The Ombudsmen does not consider it would likely find fault with how a child protection conference decided to support Mr X’s children. Also, the Ombudsmen could not achieve anything more by investigating Mr X’s complaints about York Teaching Hospital NHS Foundation Trust’s communication with him and North Yorkshire County Council.

    Summary: Ms D complained about information she received about her mother’s discharge from hospital to residential care and associated charges. The Ombudsmen have upheld Ms D’s complaints about Manchester City Council. We have not upheld her complaints about Pennine Acute Hospitals NHS Trust and Manchester University NHS Foundation Trust. The Council has accepted our recommendations, so we have completed our investigation.

    Summary: Mr X and Mr Y complained about the care their late wife and mother, Mrs F, received at Northfield Care home (the Care home) during April 2018. This was jointly arranged and funded by the Council and the Clinical Commissioning Group as Section 117 aftercare. The Council and the Clinical Commissioning Group were at fault. The care provided by the Care home to Mrs F was inadequate and it was slow to respond to Mrs F’s pressure sores. The Council was also at fault for delays in carrying out the safeguarding investigation. The Council and the Clinical Commissioning Group agreed to apologise and pay Mr X and Mr Y a total of £500 between them to acknowledge the distress and uncertainty caused by the faults.

    Summary: Mr E complains about the actions of a Council and a Trust in relation to child protection concerns and a child protection examination carried out on his son. While there were faults, I do not consider these led to the injustice that Mr E has claimed. In addition, both organisations carried out appropriate work to try and remedy the situation and prevent similar occurrences.

    Summary: The Ombudsman will not investigate Mr B’s complaint about the Council. This is because the Council has offered Mr B a reasonable explanation for closing his case and has offered him further information on how to request support in future.

    Summary: Mrs Y, and her grandson, Mr Z, complain about the way the Council and Trust handled their request for assistance when their daughter and mother, Mrs X, was due to return to the UK after being discharged from a mental health hospital abroad. The Ombudsman found fault causing injustice in the initial faulty advice provided to Mrs Y and Mr Z. However, the Ombudsmen are satisfied the Council and Trust’s subsequent actions remedied the impact of the injustice prior to the complaint to the Ombudsmen.

    Summary: Mrs X complains about the care her brother, Mr Z, received at the end of his life. The Ombudsmen have found faults in the care provided by Mr Z’s care home and an out of hours GP service. We have found no fault in the actions of Mr Z’s GP practice. The care home and out of hours GP service accept our recommendations. We have therefore completed our investigation.
