    Summary: The Ombudsmen consider Dudley Group NHS Foundation Trust’s (the Trust) communication with Mr C about Mrs D’s prognosis was poor which caused confusion and distress. Also, the Trust’s lack of care planning for Mrs D on discharge to two care homes caused Mr C uncertainty. Trinity House Care Centre’s diabetes care plan lacked detail and did not escalate Mrs D to a GP in a timely manner which caused Mr C distress at witnessing his mother suffer pain and distress. The Ombudsmen consider Dudley Metropolitan Borough Council’s (the Council) safeguarding investigation and safeguarding adults review caused Mr C uncertainty and distress. The Council, the Trust and Summerhill Surgery’s complaint handling compounded Mr C’s distress.

    Summary: The Ombudsmen do not consider NHS Devon CCG’s decision to stop Mr U’s personal health budget for his section 117 aftercare was made with fault. Devon Partnership NHS Trust’s assessment of Mr U’s needs was robust and there was no fault in the way it decided a care agency could support his needs. Also, the Ombudsmen do not consider Devon Partnership NHS Trust were at fault for not providing Mr U with a care coordinator after March 2018.

    Summary: The Ombudsmen do not consider NHS Devon CCG’s decision to stop Mr U’s personal health budget for his section 117 aftercare was made with fault.     Summary: Mr Z complains on behalf of his partner Miss X about poor care provided by a home care provider that was jointly funded by the Council and the NHS and about delay in resolving his concerns. The Ombudsmen have upheld the complaint. The Council and the care provider have agreed to apologise and provide financial redress to Miss X and Mr Z. They have also agreed to make service improvements. We have therefore completed our investigation.

The Ombudsmen did find fault with a risk assessment the Trust completed. However, we are satisfied this did not have a significant impact on the care the Trust provided.
