    Summary: Mrs A has complained about a delay in discharge of her grandmother, Mrs B, from hospital in October 2018. The Ombudsmen found fault with Walsall Trust which caused an undue delay in discharge. We did not find fault with the Council.

    Summary: The Ombudsmen finds fault by the two Councils, NELFT and the CCG in that they have failed to make appropriate educational and health provision for the complainants’ two children, who have special educational and mental health needs. The Ombudsmen have recommended steps to put things right for the children and parents. Issues about health provision for the children are now being considered by a Special Educational Needs and Disability Tribunal, which can make recommendations to NELFT about what clinical support it should provide to the children.

    Summary: The Ombudsmen find there was fault by a Trust in giving a family incorrect information about a mental health patient’s status. When this came to light it caused the patient’s wife considerable stress which has not yet been fully addressed. The Ombudsmen also find that fault by a Council meant the patient’s wife suffered this stress for too long. The Ombudsmen has recommended small financial payments to act as an acknowledgement of the outstanding injustice.

    Summary: Mrs D complained about the care and support provided to her daughter, Miss E, in a supported living placement run by Priory Care Group and jointly funded by Hampshire County Council and a clinical commissioning group. Mrs D said the response from the provider was inadequate and she wanted disciplinary action taken against those individuals she held responsible for decline in her daughter’s wellbeing. The Ombudsmen cannot add to the previous investigation completed by the authorities and cannot achieve the outcome Mrs D wants. For these reasons the Ombudsmen should not investigate the complaint.

    Summary: The Ombudsmen will not investigate Mrs X’s complaint. The Ombudsmen consider Mrs X’s complaint about events at Essex Partnership University Foundation Trust in 2017 is late, and we could not say the Trust should have detained Mr Y in 2017 and 2019. Also, the Ombudsmen do not consider there is a significant injustice to Mrs X to warrant an investigation of Southend Clinical Commissioning Group’s continuing healthcare assessment of Mr Y.
