    Summary: A woman complained about failures in her section 117 aftercare by the council, an NHS Partnership Trust, and a Clinical Commissioning Group. The Ombudsmen find failures in the way requests for direct payments were considered. Social care assessments were not completed as they should have been. Community mental health services did not properly respond to referrals by other professionals. This caused injustice, but it did not cause the woman to be without support for over a year. The organisations have agreed to take action to remedy the injustice.

    Summary: Mr A complains Lancashire County Council has not referred him for a Disability Facilities Grant. I have declined to investigate this complaint as I have not found any indication of fault on the part of the Council and there is no more an investigation could achieve for Mr A.

    Summary: The Ombudsmen find fault by the Trust, in that it sent Mr M’s follow-up appointment letters to the wrong address and as a result he was discharged from follow-up care. This meant Mr M did not have the opportunity to discuss his care with a specialist. However the Ombudsmen find it was reasonable of the Trust and Council not to refer Mr M for continuing healthcare, and that he was provided with stroke rehabilitation in line with the relevant standards and guidance.

    Summary: The Ombudsmen found no fault by a Council relating to its role in a mental health assessment. The Ombudsmen found a Trust was at fault with its communication with a patient detained under the Mental Health Act about their illness and medication. There was also fault by the Trust in relation to accessing an advocate. The Ombudsmen recommended the Trust apologises for the distress caused by the identified faults and to create an action plan to address the faults.

    Summary: Mr D complained about the actions of a Care Provider, Trust and Council when they dealt with contact the Care Provider initiated with his late mother without consent when she was in an intermediate care unit. There was fault by the Care Provider and the Trust, but they acted to improve. It is unlikely the Ombudsmen could add to the previous investigations already completed by the authorities complained about. In addition, the Ombudsmen cannot achieve the outcome the complainant wants. For these reasons the Ombudsmen should not investigate this complaint.
