    Summary

    Mr X complained the Council delayed taking action regarding his complaint of a rat infestation in and around his property. Mr X said the Council’s actions caused his family avoidable anxiety and distress and led to him moving address. We found fault by the Council. The Council has agreed to provide Mr X with an apology and a financial remedy in recognition of the injustice caused.

    Service improvements

    Remind staff to keep reports of rat and mice infestations under review in order to avoid delays in the actions taken by the Council as part of its duties under the Prevention of Damage by Pests Act 1949.

    Summary

    We found no fault on most of Ms Y’s complaints against the Council about when it removed two sets of foster children from her care. The investigation of the allegations against her was done in line with procedures, there was no fault with the decisions to remove the children, and it was entitled to hold an early household review of her fostering status which was done without fault. It accepted it failed to send her confirmation of a variation to her terms of approval as a carer and failed to ensure she received all the supervision she should have done. These failures were fault and caused her an injustice. The Council previously agreed to pay her £250. It also agreed to review why the failures happened and ensure procedures are in place and followed in the future.

    The Council agreed to review why it failed to send a copy of the exemption/variation to the number of children the complainant could foster and act to ensure these are promptly considered and sent to foster carers when made on future cases.The Council agreed to ensure there are procedures in place when social workers are off work, for foster carers to still receive the required supervision/support.

    Summary

    Mrs X complained about the Council’s failure to provide respite care for her disabled child. We found the Council to be at fault because it failed to properly consider the issue. There was also fault with the Council’s complaint handling. To remedy the distress and uncertainty caused by its faults, the Council has agreed to apologise, make a symbolic payment and take action to improve its service.

    Service improvements

    By training or other means, the Council will take action to ensure relevant complaint handling staff are aware of their responsibilities to identify complaints that should be dealt with under the statutory procedure.The Council will take action to ensure that Children in Need assessments are subject to regular review.

    Summary

    Mrs X complained about Milton Keynes Council, Milton Keynes Integrated Care Board and First Option Healthcare in relation to the care of her son, Y. She has said the faults in care led to distress for her and her family. We found fault with Milton Keynes Council, Milton Keynes Integrated Care Board and First Option Healthcare, leading to distress and risk of harm which Mrs X has described. We have made recommendations which the organisations have agreed to carry out to address this injustice.

    The Council write to the complainant outlining how it and the ICB have improved or plan to improve their commissioning and oversight of agencies providing children's continuing care at home.
