You are about to complete our online complaint form. Please only complete this form if you have already complained to the organisation involved.

There will be an opportunity to attach a copy of the final letter you have had in response to your complaint to the organisation concerned. Please do this, if you can.

You can save your form at any point (see 'save' button at the bottom of subsequent pages). You will be asked to enter your email address and a password.

Once you have submitted your form, you will be sent a copy by email shortly afterwards.

If we are already considering your complaint you should contact your investigator. If you are waiting for your complaint to be allocated to an investigator, we will be in touch as soon as we can.
