    Summary: Mrs M complains about her son B’s education. There were problems with planning for B’s transfer to post-16 education in September 2022, and the Council failed to arrange suitable provision between September 2022 and April 2023. The Council has offered a suitable remedy.

    Summary: Mrs B complained about excessive delay by the Council in issuing a final EHC plan for her son C following a needs assessment and in producing an amended EHC plan following an annual review. It also failed to properly investigate the matter when responding to Mrs B’s complaint. We have found fault causing injustice to Mrs B and C. The Council has agreed to pay Mrs B £500 for herself, and £1250 for the benefit of C’s education, in addition to completing the annual review process. The Council has also agreed to improve its procedures for the future.

    Summary: We will not investigate this complaint the Council has failed to make appropriate educational provision for the complainant’s daughter. The Council has accepted some fault and it is unlikely we could add to its response. Mrs X has now appealed to a tribunal which places the remainder of the complaint outside our jurisdiction.

    Summary: Mrs X complained about the Council’s failure to consult with her preferred school, delay in the Education, Health and Care Plan process and poor communication. We found the Council was at fault. The Council has agreed to apologise and make a payment to Mrs X to acknowledge her avoidable distress, uncertainty, time and trouble. The Council will also take action to improve its service.

    Summary: Mrs P complained about the Council’s handling of her son’s (X) education since Spring 2022 and how it communicated with her. The Council agreed it failed to find X a school placement and its alternative provision was not a full-time education. We also found it caused delay in the Education, Health and Care plan process and how it communicated with Mrs P. The Council will apologise and make payment to acknowledge the injustice it caused Mrs P and X.

    Summary: We investigated a complaint about the care provided to Mrs F. We found fault with the Nursing Home who did not keep accurate records. This caused confusion to Miss E about whether she could have avoided infection. We recommended an apology and service improvements, but the Nursing Home has closed and we cannot enforce compliance with our recommendations. We found no fault with the Council or the Trust.

    Summary: Suffolk County Council and Norfolk and Suffolk NHS Foundation Trust did not appropriately assess Miss X’s Section 117 aftercare needs in June 2022. The Council then delayed assessing her aftercare needs for 12 months. Miss X suffered distress and the fault impacted her mental wellbeing. I have recommended the Council and Trust take action to remedy the injustice to Miss X and others.
