  • Dudley Group NHS Foundation Trust (24 016 641a)

Statement Closed after initial enquiries Hospital acute services 13-May-2025

Summary: Mr X complained the NHS Trust and the Council moved his father from hospital into a care home that could not meet his needs. Mr X says the failings led to his father suffering an injury which hastened his death. Mr X also complained the Council missed carer’s assessments, which meant he missed payments. We will not investigate these complaints because there is not enough evidence of fault with the discharge planning or with how the Council dealt with a carer’s assessment request. Other complaints about earlier carer’s assessments were late.

  • London Borough of Newham (24 016 674)

Statement Closed after initial enquiries Homelessness 13-May-2025

Summary: We will not investigate this complaint about the Council’s handling of Ms X’s homelessness application because there is insufficient injustice to justify our involvement. Further, Ms X had the right to appeal to the county court in respect of the Council’s decision she was not in priority need and it was reasonable for her to exercise that right.

  • Transport for London (24 017 215)

Statement Closed after initial enquiries Parking and other penalties 13-May-2025

Summary: We will not investigate this complaint about the enforcement of penalty charge notices which were sent to the complainant’s commercial premises for a person and vehicle he did not know. The Authority has taken satisfactory action to address the complaint.

  • Durham County Council (24 017 370)

    Statement Upheld Charging 13-May-2025

Summary: We have upheld Mrs X’s complaint about a lack of advice about the costs of adult social care. The Council has agreed to apologise and make a symbolic payment to Mrs X to remedy the uncertainty caused. It will also remind relevant staff about the importance of providing appropriate information about charging for adult social care.

  • London Borough of Brent (24 018 655)

Statement Upheld Assessment and care plan 13-May-2025

Summary: We will not investigate this complaint about Mr Y’s adult social care. Part of the complaint is late. Another part relates to functions of the NHS rather than the Council, and there is insufficient evidence of fault in the Council’s actions relating to NHS functions. The Council has agreed to take early action to remedy injustice arising from fault we would likely find if we investigated the remaining complaints, and it is therefore not proportionate to investigate further.

  • City of Bradford Metropolitan District Council (24 018 678)

    Statement Upheld Charging 13-May-2025

Summary: We have upheld Ms X’s complaint about the Council’s failure to provide adequate information about the costs of adult social care for her father. We asked the Council to remedy the injustice caused. It agreed to apologise and reduce the costs it charges Mr Y. It will also take action to prevent a recurrence of this issue.

  • London Borough of Bexley (24 019 166)

Statement Closed after initial enquiries Safeguarding 13-May-2025

Summary: We will not investigate Mr X’s complaint about the Council’s investigation into a safeguarding incident. This is because the injustice is not sufficient to warrant an investigation.

  • West Lancashire Borough Council (24 002 064)

    Statement Upheld Enforcement 13-May-2025

Summary: Mr X complained that the Council failed to pursue enforcement action regarding development next to his home. There was fault by the Council due to delay and lack of communication. It has agreed a remedy

  • Staffordshire County Council (24 004 134)

Statement Upheld Special educational needs 13-May-2025

Summary: Mrs X complained the Council failed to complete an Education, Health and Care needs assessment for Y within the statutory time limits, and failed to ensure she received suitable education while she was out of school. Mrs X said this caused her real stress and meant she incurred additional expenses. We find the Council at fault. The Council has agreed to apologise, make a payment to recognise the injustice and act to prevent recurrence.

  • Telford & Wrekin Council (24 007 302)

Statement Upheld Special educational needs 13-May-2025

Summary: We have found fault with the Council for failing to secure the therapy set out in Mrs X’s daughter’s Education, Health and Care Plan. This caused her to miss out on the therapy she needed. The Council has agreed to apologise and make a symbolic payment to Mrs X in recognition of the missed provision.
