Summary: Mr X complains that Ave Maria Care Ltd failed to advise his mother, Mrs Y or her family about an increase in the hourly rate for her care. Mr X states Mrs Y did not enter into a contract with Ave Maria Care Ltd and the signature on the contract is not Mrs Y’s. The Care Provider’s failure to ensure Mrs Y was aware of and understood the cost of her care amounts to fault. This fault has caused Mrs Y an injustice.

Summary: The care provider did not provide good enough care and treatment to the late Mr X while he was resident at the care home. It failed to provide food in a way he could easily consume it and failed to administer medications properly in accordance with its own assessment. The care provider will review the way it monitors food intake and the action it takes on the results. It will apologise to Mr X’s family and offer a payment in part waiver of its fees and in recognition of the distress its failings caused.

Summary: Mrs D complains about the Care Provider's actions during the COVID-19 pandemic while her late mother was a resident at St Mary's Care Centre. We have found it was fault for the Care Provider not to facilitate an indoor end of life visit for Mrs D and her brother. This has caused significant injustice to them as they missed an opportunity to spend time with their mother before she died. The Care Provider has agreed to apologise to them.

Summary: Mr and Mrs B complained that Mr B received inadequate care during a respite placement. We found the care provider failed to provide an adequate service to Mr B causing him avoidable harm and the distress and inconvenience of an overnight hospital stay. This also caused Mrs B avoidable distress. We also find the care provider failed to keep full and accurate records about Mr B. To remedy the injustice caused, the care provider has agreed to make a payment to Mr and Mrs B.

Summary: We have not found fault with the care plan the Agency provided, the support provided or the Agency’s attempts to obtain an advocate for Mr C. However, there has been fault, during certain times, related to timekeeping, staff availability and communication. Also, the Agency’s record keeping relating to a safeguarding incident was not in line with its policy. The Agency has agreed to apologise to Mr B and Mr C and pay Mr C £150.
