    Summary: We will not investigate Mrs X’s complaint about the Council’s and NHS Trust’s decisions and communication about charges for a care placement after her mother, Mrs Y, was discharged from hospital. We found no indications of fault by the organisations with the decisions. The Trust did accept it could have given more notice on the day Mrs Y left hospital to a care home and it apologised for any distress. We are unlikely to achieve a different outcome by investigating.

    Summary: We will not investigate Ms X’s complaint that North Yorkshire Council and NHS Humber and North Yorkshire ICB failed to provide her with aftercare following her detention under the Mental Health Act in 2014. This is because a significant amount of time has passed since the events Ms X is complaining about occurred and it would have been reasonable for her to complain to us sooner. We will not investigate her complaint about the actions of an Approved Mental Health Practitioner as there is insufficient evidence of fault.

    Summary: Mrs K complained about the way Care Plus Group moved her husband, Mr H, to The Old Library Residential Home. We consider CPG acted with fault when it moved him, which caused her distress and frustration. The Old Library then did not use the right equipment to transfer Mr H. It also caused Mrs K uncertainty from its poor record keeping. The CPG and The Old Library have agreed to apologise and take action to remedy her injustice.

    Summary: Mrs K complained about the way Care Plus Group moved her husband, Mr H, to The Old Library Residential Home. We consider CPG acted with fault when it moved him, which caused her distress and frustration. The Old Library then did not use the right equipment to transfer Mr H. It also caused Mrs K uncertainty from its poor record keeping. The CPG and The Old Library have agreed to apologise and take action to remedy her injustice.

    Summary: Mr A complained that a care coordinator, working for an integrated mental health and social care service, failed to support his son to apply for Universal Credit. We have not found fault by either the responsible Council or NHS Trust. However, we have found fault in how both organisations responded to Mr A’s complaint. Neither would accept responsibility for it at first. Both organisations have agreed to apologise, take corrective action and make a financial payment to address the injustice this caused Mr A.

    Summary: Representatives for the late Mrs X complained that she did not receive the care and support she should have in a short-term residential care unit operated by Kent County Council and Kent Community Health NHS Foundation Trust. We decided not to investigate the complaint. Mrs X has died, so an investigation would not achieve a remedy for her.

However, they have accepted this and provided a suitable remedy for most of the complaint. We have recommended the Trust apologises for distress caused by fault relating to a delayed referral for therapy. We have also recommended the Trust acts to ensure safeguards are in place to prevent similar fault in future.
