    Summary: Wirral University Teaching Hospital NHS Foundation Trust acted with fault by not including Mrs X’s medication changes to her discharge paperwork. The Trust has remedied the injustice to Mrs X’s son, Dr Y. Also, Liscard Group Practice and Leighton Court Care Home did not contribute to a delay restarting Mrs X’s medication.

    Summary: Mr A complained health and social care professionals failed to adequately assess his brother’s, Mr B’s, needs before discharging him from hospital. Mr A said this meant Mr B had an inadequate care package which, in turn, placed avoidable stress and worry on the family. The Ombudsmen find no fault in the way professionals assessed Mr B’s needs.

    Summary: We found fault with the way a Council and NHS Trust handled the discharge planning process for Mr Y after his admission to hospital in April 2019. The Council and Trust agreed to apologise to Mr Y’s daughter and pay a financial sum in recognition of the distress this caused her. They will also review relevant policies and procedures to prevent similar problems occurring in future. We found no fault by another NHS Trust that cared for Mr Y during later hospital admissions in July 2019 and April 2020. Similarly, we found no fault by a GP Practice that provided care to Mr Y in the community.

    Summary: Ms Y complained about delays in her father’s discharge from hospital, poor personal care, and said he was wrongly placed in isolation. We found delays by the Trust and Council in arranging Mr Z’s discharge from hospital, leading to Mr Z remaining in hospital unnecessarily and causing distress to Ms Y. We have recommended the Trust and Council apologise to Ms Y, provide a financial remedy, and take steps to improve services.

    Summary: Mr B complained about the care and support provided to his mother when she was a patient of the Leeds Teaching Hospitals NHS Trust between October 2017 and January 2018. He also complained about the way Leeds City Council dealt with his mother’s discharge arrangements when she moved to care home from hospital in January 2018. We will not start an investigation into this complaint as we cannot add to the previous investigations completed by the Trust and the Council.

    Summary: Mr B complained about what happened when his father, Mr C, was discharged from hospital to a nursing home. We found no fault in the way the Council and Trust arranged Mr C’s discharge from hospital and communicated with his family. We note that the CCG has appropriately offered to review Mr C’s eligibility for NHS continuing healthcare.
