Report of an investigation into care provided by a care home and a nurse

The Parliamentary and Health Service Ombudsman and the Local Government Ombudsman are today publishing a report of an investigation into a complaint which identified service failures and maladministration by NHS Wakefield District Primary Care Trust and Wakefield Metropolitan District Council.

The complaint was made by a woman about the care provided to her late father at a care home funded by the Council and by a visiting community nurse from a team funded by the Trust.

This report describes service failure by the care home and the nurse and finds maladministration in the way the complaint was investigated by the Council and the Trust. It illustrates the importance of effective communication between staff involved in a person’s care and highlights the need for clear ownership of complaint handling by the organisations commissioning services.

The Local Government Ombudsman and the Parliamentary and Health Service Ombudsman are publishing this report to help others learn from the service failure and poor complaint handling it describes.
