Summary: Mr X complains about how the Council has responded to his request for a review of his housing register priority banding. He says it has not appropriately considered his medical evidence and failed to respond to his correspondence. He also complains about poor complaints handling. We have found fault in the actions of the Council for delay in actioning Mr X’s review request, reviewing and responding to correspondence and responding to his complaint. The Council has agreed to write to Mr X to apologise, pay him a financial payment and complete service improvements.

We found fault by the Council and the Trust for failings in their care planning. This led to avoidable uncertainty about whether Mrs X’s son may have missed out on effective support which could have prevented a deterioration in his mental health. We recommend that the Council and the Trust apologise, make symbolic payments and take steps to improve their services.

She says the Council failed to reassess her son as agreed, and it has failed to provide him with the specialist educational provision in his Education, Health and Care Plan. The Council was at fault for its delay in dealing with Miss B’s son’s Education, Health and Care Plan. It was also at fault for its communication with Miss B about the reassessment, and for its failure to secure some of the special educational provision in the Plan. The Council has agreed to our recommendations to address the injustice caused by fault.

Summary: Mr X complained about the care provided to his late grandfather, Mr Y at the Council commissioned care home. The care home was at fault for not properly recording communication with relatives, for not completing an accident report and for when a staff member was rude to Mr Y. The Council has agreed to apologise to Mr X and make a symbolic payment to acknowledge the distress and uncertainty caused. The Council has already taken appropriate action to prevent a recurrence of the faults.
