Summary: Miss D complained the Council stopped the taxi her son used to get to a day centre and said he should use his Motability car instead. We find some fault with how the Council carried out the transition. But we have not found fault with the Council’s decision to change the transport arrangements. The fault we have found will have caused some distress . The Council has agreed to our recommended apology and symbolic payment.

Miss X said this caused unnecessary and avoidable distress, anxiety and frustration. We find service failure and fault causing injustice. The Council will make a payment to remedy the injustice.

Summary: Ms D complains the Care Provider failed to move her father to a less expensive room, provided substandard accommodation, and did not properly care for him. The Care Provider is at fault for failing to provide satisfactory accommodation, charging for services not provided, losing items and shortfalls in care. This has caused a financial loss to the estate, uncertainty and distress to Ms D. The Care Provider has remedied some faults and following our investigation has agreed to make Ms D a symbolic payment and service improvements.

Summary: Ms C complains the Council delayed in completing an Education, Health and Care (EHC) Plan which resulted in her daughter, B, missing educational support. Ms C also complains the Council has not repaid her for a privately arranged educational psychology report and private tuition costs. The Council has accepted delays in completing an EHC Plan. It has agreed to apologise to Ms C, make a symbolic payment and repay Ms C for tuition costs and the cost of a private educational psychology report.

Summary: Ms X complains the Council failed to check the School was providing section F provisions in her daughters Education Health and Care Plan. She also said the Council did not stick to legal timeframes. The Council is at fault for failing to check the section F provisions were in place. This has caused Ms X’s daughter to miss educational provision and caused Ms X distress, time and trouble. The Council agreed to a package of remedies.

He said this meant he paid for a planning application he would not have chosen to go ahead with, and it wasted his time. We find the Council at fault and this caused injustice. The Council has agreed to apologise and make a payment to Mr X to remedy the injustice.

Summary: Ms X complained the Council failed to provide suitable education for her child following their exclusion from school in November 2022. We found fault with the Council delaying putting in place alternative provision of education from 12 September 2023 until 25 September 2023. The Council agreed to apologise to Ms X and pay her £200 in recognition of for her child’s missed education.

Summary: Ms X complained, on behalf of her son Mr Y, that the Council took too long to move Mr Y to another residential placement. Ms X said this caused Mr Y’s health to worsen. We have found the Council at fault for the time it took to move Mr Y. To remedy the injustice caused the Council agreed to apologise and make a payment to Ms X for the uncertainty she suffered.
