  • Partnership Of East London Co-Operatives (24 017 682c)

Statement Closed after initial enquiries General practice 24-Jun-2025

Summary: Mr X complained about end-of-life care for his mother, Mrs Y, from Jewish Care, Gants Hill Medical Practice, Partnership Of East London Co-Operative, NHS North East London Integrated Care Board and London Ambulance Service NHS Trust. We would likely find fault with the Practice, PELC and ICB if we investigated the complaint. Those organisations have agreed to resolve the complaint by providing a proportionate remedy for the injustice Mr X suffered.

  • Havant Borough Council (24 017 690)

Statement Closed after initial enquiries Housing benefit and council tax benefit 24-Jun-2025

Summary: We will not investigate this complaint about Mr X’s claim for a discretionary housing payment. This is because investigation would not lead to a different outcome.

  • Halton Borough Council (24 017 778)

Statement Closed after initial enquiries Antisocial behaviour 24-Jun-2025

Summary: We will not investigate this complaint about the Council’s delay in responding to a request for an anti-social behaviour case review and its failure to explain why the request did not meet the threshold for review. The Council has apologised for delay. It has also confirmed it will explain its reasons going forward. We consider further investigation will not lead to a different outcome.

  • Gloucestershire County Council (24 017 849)

    Statement Upheld School transport 24-Jun-2025

Summary: Miss B complained the Council failed to backdate her enhanced personal budget for transport to December 2024 and that caused her financial hardship. The Council has now agreed to backdate the personal budget to December 2024. There is nothing further the Ombudsman can achieve for Miss B.

  • London Borough of Southwark (24 017 918)

    Statement Upheld Other 24-Jun-2025

Summary: Ms X complained the Council failed to help her family including support for her daughter with suicidal ideation, not providing suitable education and leaving the family in a house where they were at risk. The Council was at fault for failing to consider the complaint under the children’s statutory complaints procedure. It will now consider Ms X’s concerns through the statutory procedure as well as apologising and making a payment for failing to do this originally.

  • London Borough of Croydon (24 018 050)

Statement Upheld Special educational needs 24-Jun-2025

Summary: The Council was at fault as it failed to provide occupational therapy for Miss X’s child from September 2024 to March 2025 in line with their Education, Health and Care Plan, and it poorly communicated and handled her case. It will apologise and make a payment to Miss X to acknowledge the impact of the missed occupational therapy provision. It will also identify why it failed to arrange the occupational therapy provision and report back to us on the action it has taken to address this.

  • Essex County Council (24 019 084)

Statement Upheld Special educational needs 23-Jun-2025

Summary: Mrs X complained the Council did not complete her child’s Education, Health and Care needs assessment within statutory timescales and it refused to reimburse the cost of a private Educational Psychologist report. We found the Council at fault. It will apologise to Mrs X, pay her £500 to recognise the avoidable frustration and uncertainty caused by a delay in completing the needs assessment process and reimburse the cost of the Educational Psychologist report.

  • Lancaster City Council (24 019 744)

Statement Closed after initial enquiries Antisocial behaviour 23-Jun-2025

Summary: We will not investigate Ms X’s complaint about the Council’s handling of her complaint about noise nuisance. There is insufficient evidence of fault and it is unlikely an investigation would reach a different outcome.

  • Mid Devon District Council (24 022 252)

Statement Closed after initial enquiries Drainage 23-Jun-2025

Summary: We will not investigate this complaint about planning permission and drainage because the complaint is late, without good reason to exercise discretion to investigate and it is reasonable to expect Mrs Y to go to court about the matter, which is better placed to consider the complaint.
