    Summary: The Ombudsmen found fault with the care provided to a woman with complex needs by several health and social care organisations. This included failure to arrange assessments and reviews, failures in transition planning and failure to make appropriate safeguarding referrals. This fault meant opportunities were missed to explore whether the woman had additional care needs. This also resulted in distress and uncertainty for her sister, who is pursuing the complaint on her behalf. The organisations concerned have agreed to take action to remedy the injustice to the complainants.

    Summary: Miss X complained about her father being in a nursing home, and the care he receives there. She also complained about the Council having taken steps towards selling the family home. We cannot investigate this complaint. We already considered part of the complaint in 2015 and we cannot investigate that again. Other issues are being considered in court and are not for the Ombudsman.

    Summary: We will not investigate Mr and Mrs X’s complaint that an employee of the care agency, who supports their neighbour, swore at them and used foul language. The Care Provider’s apology is a proportionate remedy for their injustice.

    Summary: Mrs B says the Council unreasonably cancelled her son’s transport without assessing the suitability of alternative transport arrangements and an officer was rude and unhelpful. I cannot reach a safe conclusion about how an officer dealt with Mrs B. For the transport arrangements the Council misunderstood what arrangements were in place when it arranged for a support worker to accompany Mrs B’s son to and from a provision. That left him stranded at the provision and Mrs B had to make alternative arrangements for the following week. An apology, payment to Mrs B and her son and agreement to confirm any changes to transport arrangements in writing is satisfactory remedy for the injustice caused.

    Summary: The Ombudsman will not investigate this complaint about a holiday arranged by the Council. This is because the Council has already provided a fair remedy and it is unlikely an investigation would lead to a different outcome.
